At Kotra Pharma we take the safety of our patients very seriously. If you noticed or if you think you may have a health problem related to a Kotra Pharma medicine or any other Kotra Pharma health supplement product, you can contact us at any time and report it to us by filling out the form below.
To help us process your information quickly and effectively, please report as much relevant information as possible. At a minimum you will need to provide the following:
1. The initials / identifiers of the person who experienced the possible side effect, gender, date of birth, ethnicity.
2. The name of the Kotra Pharma product.
3. A description of the possible side effect, such as the signs and symptoms experienced, date the possible side effect started and the outcome of the possible side effect (fully recovered / recovering / not recovered / fatal).
4. The contact information of the person reporting the issue.
